Advocate Health, the third-largest nonprofit integrated health system, is seeking a primary care physician who enjoys nursing home care, to grow our geriatric program as a Skilled Nursing Facility (SNF) Physician in Oshkosh, Wisconsin.

- Join a team of two physicians and two NP’s in our SNF’ist program in Oshkosh and Fond du Lac
- Incoming clinician will primarily be covering three facilities in Oshkosh and two facilities in Fond du Lac on occasion
