Advocate Health is recruiting for Family Medicine or Emergency Medicine Physicians to join our team in a transition of care role in Oshkosh, Wisconsin, a safe, family friendly community that offers limitless recreational activities.
Job details
- Newly created position to reduce readmissions and repeated urgent care and emergency room visits
- Clinician will see patients 7 to 14 days after discharge as well as do Pre-ops; APC will do follow ups from the emergency department and urgent care
- Easy access to subspecialty providers as well primary care referrals for your patients within our system
- Office hours Monday through Friday from 8:30 a.m. to 5:30 p.m.
- Employed outpatient only position with comprehensive benefits package, competitive hourly rate, fully funded malpractice insurance, CME allowance, relocation assistance, and student loan forgiveness
